Program Steps

  1. Gather Required Documentation: Prepare the necessary documents for your application. These include:

    • Business registration details
    • Financial statements (last two years)
    • Business plan
    • Proof of insurance
    • Tax compliance verification
    • Attestation of compliance with labour and environmental laws
  2. Complete the SE Employer Registration Form (3004E): Fill out this form to register your business for the program.

  3. Complete the SE Financial Support Agreement (3045E): This agreement outlines the financial supports available and the terms of your participation.

  4. Submit Your Application: Send your completed forms and documentation to the designated Employment Ontario office in your catchment area (Belleville, Cornwall, or Timmins).

  5. Review and Follow-Up: After submission, the Employment Ontario office will review your application. Be prepared to provide any additional information or clarification if requested.

  6. Attend Training Sessions: Participate in any required training sessions, such as the Supported Employment (SE) Phase 1 Technical Training.

  7. Await Approval and Next Steps: Once your application is approved, you will receive further instructions on how to proceed with the program and access the support services available.
